quick help with visa form

i have a quick question on the q1 visa form doc ds-156

on part 25 - it asks who you will be staying with and their address and phone number - anyone know if we have to fil this in or can leave it blank?if we do have to fill it in what goes there. for where i am staying i have put the address for the commons housing complex off our luggage tags

i received the q1 visa checklist that just says to fill in the form online and pay and stuff and the I-797 form - cant find any other info anywhere I've looked through my original disney pack too and the stuff sent then and i still cant find it -maybe im being dense ???

 

iv no idea what a q1 is but if its the same as we needed to fill out on the 156 for a j1 visa then it was the name of the person who is stated on your ds2019. mine was just a random person who i dont know lol! also when yummys emailed u info about applying for the visa, there was a help section on the sheet.

 

I didn't get a help thing either, oh well here goes nothing....

yes, you do have to fill this part in, very important, the us embassy needs to know who and where you'll be staying!!
